    Phase 1Weeks 1–3

    Diagnostic

    Workforce data collection, ARRS utilisation review, mandatory training and HR compliance audit, appointment data analysis for skill mix, sickness and retention review, conversations with practice manager and clinical lead.

    Deliverable: Workforce Diagnostic Report — current state across workforce, compliance, skill mix and development.

    Phase 2Weeks 4–5

    Planning

    12-month workforce plan drafted, skill mix recommendations developed, HR compliance action plan produced, development and succession framework drafted, partner review and sign-off.

    Deliverable: Workforce Programme Plan — agreed priorities, actions, owners and 90-day milestones.

    Phase 3Weeks 6–12

    Implementation

    HR compliance remediation underway, skill mix changes initiated, appraisal cycle introduced and first round scheduled, development plans in place for key roles, workforce plan shared with partners and PCN lead where relevant.

    Deliverable: Implementation Progress Report at Week 12.

    Phase 4Weeks 13–16

    Embed & Evidence

    Mandatory training compliance at or trending toward 90%+, workforce plan reviewed for remainder of year, development plans reviewed with practice manager, HR compliance position documented for CQC readiness.

    Deliverable: Programme Close Report — compliance position change, skill mix changes implemented, workforce plan summary.
